Approval of Amendment No. 2 to Intergovernmental Agreement Contract No. CTR055258 with the Arizona Department of Health Services (ADHS) for Title V Maternal and Child Health (Healthy Arizona Families) in the amount of $151,325 ($102,673 for MCH Healthy Arizona Families and $48,652 for Public Health Improvement (PHI) Program).
Background Information
The initial Intergovernmental Agreement Contract No. IGA202-040 was approved by the Board of Supervisors on September 15, 2020, whereby the Arizona Department of Health Services provided $102,673 to the Health and Emergency Management Department to provide Title V Maternal and Child Health, Healthy Arizona Families Program services for the period of July 1, 2020, to June 30, 2025.

Amendment No. 1 changed the contract number from IGA2020-040 to CTR55258 and revised the Priced Sheet which extended the contract for the period of July 1, 2021, to June 30, 2022, for the amount of $102,673.

Amendment No. 2 adds the Public Health Improvement (PHI) Program to this IGA as of July 1, 2022. Under the PHI Program, Gila County Health Department will implement evidence-based/evidence-informed strategies at the local community level that: Promote and implement healthy community interventions that target policies, systems, and environmental approaches that will shape the communities in which we live, learn, work, and play; and promote and implement healthy people interventions that target individual behavior and support making healthy choices.
Evaluation
Amendment No. 2 to Contract No. CTR055258 includes a revised price sheet for funding the Title V Maternal and Child Health, Healthy Arizona Families in the amount of $102,673 and funding for the PHI Program in the amount of $48,652.

This funding will continue the County's public partnerships between the Arizona Department of Health Services and the Gila County Health Department by providing Title V MCH Block Grant funding to support the implementation of health priorities identified through Arizona's Statewide Needs Assessment and MCH statewide needs assessment.

Amendment No. 2 to Contract No. CTR055258 amends the contract, as follows:
1. Includes the Public Health Improvement (PHI) Program Scope of Work.
2. Updates the price sheet to include Public Health Improvement (PHI) Program funding in the amount of $48,652.
3. Adds Exhibit B - 2 CFR 200.332, Exhibit C - 2 CFR 200.332, Exhibit D - 2 CFR 200.332 - Requirements for pass-through entities.
Conclusion
Approval of Amendment No. 2 to Contract No. CTR055258 in the amount of $151,325 will extend the current Maternal and Child Health program funding to June 30, 2023, while incorporating the Public Health Improvement (PHI) Program.
Recommendation
The Health and Emergency Management Department Director recommends that the Board of Supervisors approve Amendment No. 2 to Contract No. CTR055258 in the amount of $151,325 for July 1, 2022, to June 30, 2023.
