Approval of Amendment No. 5 to an Intergovernmental Agreement (Contract No. CTR043241) with the Arizona Department of Health Services (ADHS) to provide funding to be used towards special projects to enhance program integrity, WIC Services, or WIC Facilities.

Background Information

The initial Intergovernmental Agreement was signed by the Board of Supervisors on February 5, 2019, as Contract No. ADHS19-207418.

Amendment No. 1 to that contract changed the contract number from ADHS19-207418 to CTR043241 and it was approved by the Board on August 19, 2019.

Amendment No. 2 replaced the scope of work and the price sheet, and the contract name was changed to WIC and BFPC Services.

Amendment No. 3 replaced the scope of work to include a general infrastructure special project to renovate, expand, and improve County-owned property in Payson, Arizona which will improve capacity, nutrition, and breastfeeding services. The price sheet was also replaced.

Amendment No. 4 replaced the scope of work and price sheet, and Exhibits A-C were added.

Amendment No.5 replaced the scope of work to include a special project to replace the Globe WIC building flooring. The price sheet was also replaced.

Evaluation

Amendment No. 5 needs to be approved by the Board of Supervisors for the special projects funding to replace the Globe WIC building flooring.

Conclusion

This funding will allow the Gila County WIC to replace the Globe WIC office building flooring. This will improve the facility and improve staff and client satisfaction by creating a clean inviting environment.

Recommendation

It is the recommendation of the Health and Emergency Management Department Director that the Board of Supervisors approve Amendment No. 5 to the Intergovernmental Agreement (Contract No. CTR043241) with ADHS.

Suggested Motion

Approval of Amendment No. 5 to an Intergovernmental Agreement (Contract No. CTR043241) with the Arizona Department of Health Services in the amount of $14,459.00 for the Special Project for WIC (Women, Infants, and Children) and BFCP (Breastfeeding Peer Counseling) services to replace the Globe clinic flooring.
