Amendment No.1 for Contract No. CTR055335 (formerly IGA2020-017) Gila County - Healthy People Healthy Communities
Background Information
The Arizona Department of Health Services (ADHS) has integrated multiple grants into one Intergovernmental Agreement (IGA) and the title of the new integrated contract is called "Healthy People Healthy Communities." Implementation of this IGA was completed in three (3) phases. Phase 1: The Tobacco Free, Chronic Disease, and Health in Arizona Policy Initiative (HAPI) grants were integrated and implemented on July 1, 2015; Phase 2: The Public Health Services block grants were integrated and implemented on October 1, 2015; and, Phase 3: The Teen Pregnancy Prevention, Family Planning, and the Maternal and Child Health grants were integrated and completed on June 30, 2016. The original Contract No. ADHS15-094962 was signed by the Board of Supervisors on June 9, 2015, and then due to a system change at ADHS, the contract number was changed to No. ADHS16-098369 and approved by the Board of Supervisors on September 15, 2015, under Amendment No. 1. It integrated the Phase 3 grants into one contract entitled "Healthy People Healthy Communities." The total amount of the new contract is $360,682.

Amendment No. 2 was approved by the Board at its February 16, 2016, regular meeting. It increased the total grant award by $13,124, raising the total contract to $373,806.

Amendment No. 3 was approved by the Board at its May 3, 2016, regular meeting. It continued the integration of grant funding into one contract and replaced several sections, exhibits, and price sheets in the original contract with those identified in this amendment.

Amendment No. 4 was approved by the Board on October 18, 2016. This increased the contract amount with ADHS in the amount of $47,090.

Amendment No. 5 was approved by the Board on July 18, 2017, which initiated participation in Healthy Arizona Worksite Program (HAWP). This amendment allowed the Gila County Public Health Division to implement influenza vaccinations at the Gila County Department of Health and Emergency Management worksite.

Amendment No. 6 was approved by the Board on December 5, 2017, and included a revision to the IGA price sheet for the Preventive Health and Health Services Block grant in the amount of $36,630.

Amendment No. 7 was approved by the Board on August 7, 2018, which reflected the addition of one-quarter of $12,210 for the accreditation program and an updated scope of work.

Amendment No. 8 was approved by the Board on December 18, 2018, and included a revised price sheet for three additional quarters of public health accreditation work.

Amendment No. 9 was approved by the Board on September 17, 2019, and a price sheet that included four additional quarters of funding for the Health in Arizona Policy Initiative in the amount of $56,649.36.

Beginning in July 2020, the original HPHC IGA has split apart and two (2) separate IGAs have formed; a fixed price contract and a cost-based reimbursement contract. The programs to be included in this fixed-price contract are: Tobacco Cessation and Prevention, Teen Pregnancy Prevention and its Youth Mental Health First Aid Initiative, Health in Arizona Policy Initiative (HAPI), Public Health Improvement, and the Suicide Mortality Review Programs.

The Suicide Mortality Review Program is administered by ADHS, which will develop a suicide mortalities data collection system, encourage and assist in the development of local suicide mortality review teams, and provide training and technical assistance to those teams. The purpose of this IGA is to leverage multiple public health funding sources to support the implementation of health priorities identified in the AzHIP (2021- 2026) and the County Health Improvement Plans (CHIP). This IGA is intended to provide flexibility to the County Health Departments to best meet the needs of their local communities through high-impact strategies that realize the agreed-upon outcomes. The IGA provides a pathway to improved coordination of multiple prevention programs and the Suicide Mortality Review Program while streamlining the administrative functions for the programs that were previously administered separately.
Evaluation
The Gila County Health and Emergency Management Department will implement evidence-based strategies at the local community level that:
  • Promote and implement healthy communities’ interventions that target policy, system, and environmental approaches that will shape the communities in which we live, learn, work, and play; and
  • Promote and implement healthy people interventions that target individual behavior and support making healthy choices.
The County Contractor shall implement evidence-based strategies and conduct Suicide Mortality Review at the local community level that:
  • Promote and implement healthy communities' interventions that target policy, system, and environmental approaches that will shape the communities in which we live, learn, work, and play;
  • Attend ADHS training and technical assistance sessions on standards and protocols for local suicide mortality review teams;
  • Bring together local community agencies in a formal process to systematically share information on suicide events for persons over the age of eighteen (18) years old, identify risk factors in those deaths, and provide prevention recommendations.
Conclusion
Approval of Amendment No. 1 for Contract No. CTR055335 (formerly IGA2020-017) Gila County - Healthy People Healthy Communities will further efforts to impact policy, system, and environmental change at the community, organizational, individual, and policy levels in order to promote county-wide health changes so that public health impact will be maximized.
Recommendation
It is the recommendation of the Health and Emergency Management Department Director that the Board of Supervisors approve Amendment No. 1 for Contract No. CTR055335 (formerly IGA2020-017) Gila County - Healthy People Healthy Communities. This will further our efforts to impact policy, system, and environmental change at the community, organizational, individual, and policy levels in order to promote county-wide health changes so that public health impact will be maximized.
Suggested Motion
Information/Discussion/Action to approve Amendment No. 1 to Contract No. CTR055335 (formerly IGA2020-017) Gila County - Healthy People Healthy Communities in the amount of $443,715 beginning July 1, 2021, through June 30, 2022, that is between the Arizona Department of Health Services and Gila County Health and Emergency Management Department which replaces Contract No. IGA20720-01 to continue providing services under the Healthy People, Healthy Communities Program.
