| Background Information |
On February 4, 2020, the Board of Supervisors awarded Professional Services Contract No. 102119-Health Prevention and Surveillance Services to Guild Consulting in the amount of $100,000.
On August 4, 2020, the Board of Supervisors approved Amendment No. 1 to Professional Services Contract No. 102119-Health Prevention and Surveillance Services to extend the contract term to align with the fiscal year from July 1, 2020, to June 30, 2021, for an amount not to exceed $100,000.
On June 15, 2021, the Board of Supervisors approved Amendment No. 2 to Professional Services Contract No. 102119-Health Prevention and Surveillance Services to extend the contract term to align with the fiscal year from July 1, 2022, to June 30, 2023, for an amount not to exceed $100,000. |
