Amendment No. 2 to Professional Services Contract No. 102119-Health Prevention and Surveillance Services.

Background Information

On February 4, 2020, the Board of Supervisors awarded Professional Services Contract No. 102119-Health Prevention and Surveillance Services to Guild Consulting in the amount of $100,000.

On August 4, 2020, the Board of Supervisors approved Amendment No. 1 to Professional Services Contract No. 102119-Health Prevention and Surveillance Services to extend the contract term to align with the fiscal year from July 1, 2020 to June 30, 2021, for an amount not to exceed $100,000.

Evaluation

Amendment No. 1 to Professional Services Contract No. 102119-Health Prevention and Surveillance Services expires on June 30, 2021. The contract allows for three additional one-year renewal periods; therefore, staff recommends that the contract be renewed for one additional year, from July 1, 2021 to June 30, 2022, for an amount not to exceed $100,000.

Conclusion

The Health & Emergency Management Department Director wishes to execute Amendment No. 3 to Professional Services Contract No. 102119 with Guild Consulting to extend the contract term from July 1, 2021 to June 30, 2022.

Recommendation

The Health & Emergency Management Department Director recommends approval of Amendment No. 2 to Professional Services Contract No. 102119-Health Prevention and Surveillance Services with Guild Consulting to extend the contract term from July 1, 2021 to June 30, 2022, for a contract amount not to exceed $100,000.
