Background Information |
On December 18, 2018, the Board of Supervisors awarded Contract No. 091718 - Sub-Grantee for Public Health Preparedness to Guild Health Consulting in the amount of $115,000.
On August 19, 2019, the Board of Supervisors approved Amendment No. 1 to Professional Services Contract No. 091718-Public Health Preparedness to extend the contract term to align with the Public Health Emergency Preparedness Grants FFY from July 1, 2019, to June 30, 2020; and to increase the dollar amount by $12,000 for a new contract amount of $127,000.
On July 8, 2020, the County Manager approved Amendment No. 2 to Professional Services Contract No. 091718-Public Health Emergency Preparedness extending the term of the contract for one additional year from July 1, 2020, to June 30, 2021. |
Evaluation |
Amendment No. 2 to Professional Services Contract No. 091718-Sub-Grantee for Public Health Preparedness expires on June 30, 2021. The contract allows for three additional one-year renewal periods; therefore, staff recommends that the contract be renewed for one additional year, from July 1, 2021 to June 30, 2022, with a not to exceed contract amount of $127,000. |
