Amendment No: 1 to an Intergovernmental Agreement (Contract No. IGA2020-040 IGA) with the Arizona Department of Health Services (ADHS) for Title V Maternal and Child Health (Health Arizona Families) in the amount of $102,673.

Background Information

The initial Intergovernmental Agreement (Contract No. IGA202-040) was approved by the Board of Supervisors on September 15, 2020 whereby the Arizona Department of Health Services provided $102,673 to the Health and Emergency Management Department to provide Title V Maternal and Child Health Healthy Arizona Families program services for the period of July 1, 2020, to June 30, 2025.

Evaluation

Amendment No. 1 to Contract No. IGA2020-040 includes a revised price sheet for funding the Title V Maternal and Child Health Healthy Arizona Families in the amount of $102.673.

This funding will continue the County's public partnerships between ADHS and the Gila County Health Department by providing Title V MCH Block Grant funding to support the implementation of health priorities identified through Arizona's Statewide Needs Assessment and MCH statewide needs assessment.

Amendment No. 1 to Contract No. IGA202-040 amends the contract, as follows:
1. The contract number is revised from IGA202-040 to CTR055258 due to the contracts being placed back in the Arizona Procurement Portal.
2. The Scope of Work is revised and replaced by the Scope of Work of this Amendment One.
3. The Price Sheet is revised to include the Price Sheet of this Amendment One.
4. Attachment H - ADHS Family Planning Policy and Procedure Manual has been removed.
5. Exhibit A has been added.

Conclusion

Approval of Amendment No. 1 to Contract No. IGA2020-040 in the amount of $102,673 will extend the current Maternal and Child Health program funding through June 30, 2025.

Recommendation

It is the recommendation of the Health and Emergency Management Department Director that the Board of Supervisors approve Amendment No. 1 to Contract No. IGA2020-040 in the amount of $102,673 for July 1, 2021 through June 30, 2022.

Suggested Motion

Information/Discussion/Action to approve Amendment No. 1 to an Intergovernmental Agreement (Contract No. IGA2020-040) between the Arizona Department of Health Services and Gila County Health Division for the funding of Title V Maternal and Child Health Healthy Arizona Families Initiative to change the contract number to CTR055258, and to revise the Scope of Work and the Price Sheet which extends the contract for the period of July 1, 2021, through June 30, 2022, in the amount of $102,673.
