Grant Agreement No. RFGA2020-001-04 and Amendment No. 1 to said Grant Agreement for AZ Health Zone (SNAP-Ed) between the Gila County Health and Emergency Management Department and the Arizona Department of Health Services.
Background Information
Since 2015, the Gila County Health and Emergency Management Department has applied for and received grant funding from the Arizona Department of Health Services (ADHS) to develop and implement comprehensive nutrition and physical activity programs. These programs attempt to address local needs related to nutrition, physical activity, and obesity prevention by utilizing evidence-based strategies.

In 2015, the grant contract was initially awarded as number ADHS15-00004836 for the period October 1, 2015, through September 30, 2018. Effective October 1, 2017, the grant was extended for two additional one-year increments or two successive years not to exceed a total grant term of five years. Additionally, effective October 1, 2017, the grant contract name was changed from AzNN (Arizona Nutrition Network) SNAP-Ed (Supplemental Nutrition Assistance Program Education) Local Implementation Services to Arizona Health Zone (SNAP-Ed) Local Implementation Services and the contract number was changed to ADHS16-106556.

In June 2020, the Gila County Health and Emergency Management Department electronically submitted a grant application to ADHS to obtain additional funds to continue providing these services. The Board Chairman signed the grant application before it was presented to the Board of Supervisors for approval due to the short turnaround time to submit the grant application. The intent was to request ratification of the Board of Supervisors' approval to submit the grant application; however, staff inadvertently forgot to place the item on the next Board meeting agenda.

On July 27, 2020, ADHS awarded Grant Agreement No. RFGA2020-001-04 in the amount of $224,030.40 for a performance period of October 1, 2020, through September 30, 2021. (This grant replaced Grant Agreement No. ADHS16-106556.) Due to the demands of staff during the Covid-19 pandemic, staff inadvertently forgot to prepare an agenda item for the Board to accept the grant award. The Grant Agreement with the Chairman's signature needs to be ratified because it was pushed through the electronic system to continue to provide these services in Gila County under Contract No. RFGA2020-001-04.

Amendment No. 1 to Contract No. RFGA2020-001-04 will extend the contract through September 30, 2022.
Evaluation
To have an accurate record of this grant, the Board of Supervisors needs to ratify its approval of Grant Agreement No. RFGA2020-001-04 in the amount of $224,030.40, and approve Amendment No. 1 to said Grant Agreement to extend the term of the agreement for an additional year to September 30, 2022.

Acceptance of this funding and extension of the performance period will enable the Gila County Health and Emergency Management Department to continue to implement a policy, systems, and environmental change focus that makes healthier choices a real, feasible option for every community member by looking at the laws, rules, and environments that impact our behavior. Policy, systems, and environmental change are ways of modifying the environment to make healthy choices practical and available to all community members.
Conclusion
By ratifying the Board's approval of Grant Agreement No. RFGA2020-001-04, and approving Amendment No. 1 to Grant Agreement No. RFGA2020-001-04 to extend the agreement to September 30, 2022, it will allow the Gila County Health and Emergency Management Department to continue providing these services in Gila County. In addition, obtaining the Board's approval will formalize this agreement with ADHS.
Recommendation
It is the recommendation of the Health and Emergency Management Department Director that the Board of Supervisors ratify its approval of Grant Agreement No. RFGA2020-001-04; and approve related Amendment No.1 to continue to provide AZ Health Zone (SNAP-Ed) services through September 30, 2022.

Information/Discussion/Action to ratify the Board of Supervisors' approval of Grant Agreement No. RFGA2020-001-04 with the Arizona Department of Health Services in the amount of $224,030.40; and approve Amendment No. 1 to said agreement to extend the performance period to September 30, 2022, which will allow the Gila County Health and Emergency Department to continue providing AZ Health Zone Program (SNAP-Ed) services.
