Approval of Agreement No. 12022022 with Horizon Health and Wellness

Background Information

Horizon Health and Wellness is a non-profit organization that provides transportation, medical, behavioral health, and social services for children and adults with serious mental illness in the Globe/Miami/San Carlos areas. Horizon is a non-profit integrated healthcare provider licensed in Arizona and accredited through the Joint Commission to deliver a wide variety of inpatient, outpatient, and residential services for all ages and stages of life. Their comprehensive healthcare services include family medicine, emotional and behavioral health, addiction treatment, psychiatry, counseling, Telehealth, and more.

Evaluation

Horizon Human Services provides a wide variety of services for children and adults with serious mental illness in the Globe/Miami/San Carlos areas.

Conclusion

It is the intent of the County, pursuant to A.R.S. §11-267, to provide $1,000 to Horizon Health and Wellness. The purpose of these funds will be to provide services to persons with disabilities which are operated and maintained within the boundaries of the County and have been determined to be for the benefit of the public per A.R.S. §11-267.

Recommendation

Staff recommends that the Board of Supervisors approve Agreement No. 12022022 between Gila County and Horizon Health and Wellness in the amount of $1,000.

Suggested Motion

Information/Discussion/Action to approve Agreement No. 12022022 with Horizon Health and Wellness whereby the County will disburse $1,000; and further, the Board finds and determines this is for the benefit to provide services to children and adults with serious mental illness in the Globe/Miami/San Carlos areas.
