Request/Subject
Approval of Amendment No. 6 to an Intergovernmental Agreement (Contract No. CTR043241) with the Arizona Department of Health Services (ADHS) to continue providing Women, Infant, and Child (WIC) Services.
Background Information
The initial Intergovernmental Agreement was signed by the Board of Supervisors on February 5, 2019, as Contract No. ADHS19-207418.

Amendment No. 1 to that contract changed the contract number from ADHS19-207418 to CTR043241 and it was approved by the Board on August 19, 2019.

Amendment No. 2 replaced the scope of work and the price sheet, and the contract name was changed to WIC and Breastfeeding Peer Counseling (BFPC) Services.

Amendment No. 3 replaced the scope of work to include a general infrastructure special project to renovate, expand, and improve County-owned property in Payson, Arizona which will improve capacity, nutrition, and breastfeeding services. The price sheet was also replaced.

Amendment No. 4 replaced the scope of work and price sheet, and Exhibits A-C were added.

Amendment No.5 replaced the scope of work to include a special project to replace the Globe WIC building flooring. The price sheet was also replaced.

Amendment No. 6 replaces the scope of work, price sheet, and Exhibit A and B from the original contract. The funding formula for WIC is based on the number of clients served and due to the decline in birth rates and a decrease in clients not coming to WIC across the Nation the budget will be reduced in Gila County by $41,088. The budget is based on caseload which has been reduced to 900 per month from 1100 per month.
Evaluation
Amendment No. 6 needs to be approved by the Board of Supervisors to continue to provide WIC and BFPC services in Gila County.
Conclusion
This funding will allow the Gila County WIC to continue to provide nutrition and breastfeeding support services to Gila County families.
Recommendation
The Health & Emergency Management Department Director recommends that the Board of Supervisors approve Amendment No. 6 to the Intergovernmental Agreement (Contract No. CTR043241) with ADHS.
