Approval of Amendment No. 9 to an Intergovernmental Agreement (Contract No. ADHS16-098369) with the Arizona Department of Health Services (ADHS) to revise the price sheet to include 4 additional quarters of funding for the Health in Arizona Policy Initiative in the amount of $56,649.36.
Background Information
The ADHS has integrated multiple grants into one Intergovernmental Agreement (IGA) and the title of the new integrated contract is called "Healthy People Healthy Communities."  Implementation of this IGA was completed in three (3) phases.

Phase 1: The Tobacco Free, Chronic Disease, and Health in Arizona Policy Initiative (HAPI) grants were integrated and implemented on July 1, 2015;

Phase 2: The Public Health Services block grants were integrated and implemented on October 1, 2015; and,

Phase 3: The Teen Pregnancy Prevention, Family Planning, and the Maternal and Child Health grants was integrated and completed on June 30, 2016.

The original Contract No. ADHS15-094962 was signed by the Board of Supervisors on June 9, 2015, and then due to a system change at ADHS, the contract number was changed to No. ADHS16-098369 and approved by the Board of Supervisors on September 15, 2015.

Amendment No. 1 was approved by the Board at its December 1, 2015, regular meeting. It integrated the Phase 3 grants into one contract entitled "Healthy People Healthy Communities."  The total amount of the new contract is $360,682 and is being administered by the Gila County Public Health Division.

Amendment No. 2 was approved by the Board at its February 16, 2016, regular meeting. It increased the total grant award by $13,124, raising the total contract to $373,806. This additional funding allowed Gila County to assist ADHS in administering a survey to more clearly understand the experience of families in meeting their children's special needs across the state.

Amendment No. 3 was approved by the Board at its May 3, 2016, regular meeting.  It continued the integration of grant funding into one contract and replaced several sections, exhibits, and price sheets in the original contract to those identified in this amendment. This allowed the Gila County Health and Emergency Services Department to continue to provide essential services to our communities.

Amendment No. 4 was approved by the Board on October 18, 2016.  This increased the contract amount with ADHS in the amount of $47,090 to continue preparing for national accreditation.

Amendment No. 5 was approved by the Board on July 18, 2017, which initiated participation in Healthy Arizona Worksite Program (HAWP). This amendment allowed the Gila County Public Health Division to implement influenza vaccinations at the Gila County Department of Health and Emergency Management worksite.

Amendment No. 6 was approved by the Board on December 5, 2017, and included a revision to the IGA price sheet for the Preventive Health and Health Services Block grant in the amount of $36,630. This allowed the Gila County Department of Health and Emergency Management to continue working towards national accreditation.

Amendment No. 7 was approved by the Board on August 7, 2018, which reflected the addition of one quarter of $12,210 for the accreditation program and an updated scope of work.

Amendment No. 8 was approved by the Board on December 18, 2018, and included a revised price sheet for 3 additional quarters of public health accreditation work.
Evaluation
Amendment No. 9 to Contract No. ADHS16-098369 includes a revised price sheet that includes 4 additional quarters of funding for the health in Arizona Policy initiative in the amount of $56,649.36. This funding will further our public health work in the areas of Alzheimer's, Chronic Pulmonary Disease, Hypertension, Disease Self-Management, Healthy Community Design, School Health, Worksite Wellness, Clinical Care, and Special Health Care Needs.
Conclusion
Approval of Amendment No. 9 to Contract No. ADHS16-098369 to revise the price sheet to include 4 additional quarters of funding for the Health in Arizona Policy Initiative in the amount of $56,6449.36. This funding will further our public health work in the areas of Alzheimer's, Chronic Pulmonary Disease, Hypertension, Disease Self-Management, Healthy Community Design, School Health, Worksite Wellness, Clinical Care, and Special Health Care Needs.
Recommendation
It is the recommendation of the Health and Emergency Management Department Director that the Board of Supervisors approve Amendment No. 9 to Contract No. ADHS16-098369 to revise the price sheet to include 4 additional quarters of funding for the Health in Arizona Policy Initiative in the amount of $56,6449.36. This funding will further our public health work in the areas of Alzheimer's, Chronic Pulmonary Disease, Hypertension, Disease Self-Management, Healthy Community Design, School Health, Worksite Wellness, Clinical Care, and Special Health Care Needs.
