Amendment No. 1 to Intergovernmental Agreement (new Contract No. CTR043241) with Arizona Department of Health Services (ADHS).

Background Information

This original Intergovernmental Agreement was signed by the Board of Supervisors on February 5, 2019, as Contract No. ADHS19-207418. Amendment No. 1 to that contract changes the contract number from ADHS19-207418 to CTR043241 due to change in the electronic system.

Evaluation

Amendment No. 1 changes the contract number from ADHS19-207418 to CTR043241 due to a change in the electronic system.

Conclusion

This funding will allow the Gila County Health Department to continue providing nutrition services to qualified families.

Recommendation

It is the recommendation of the Health and Emergency Management Department Director that the Board of Supervisors approve Amendment No. 1 to the Intergovernmental Agreement (Contract No. CTR043241) with ADHS.

Suggested Motion

Approval of Amendment No. 1 to an Intergovernmental Agreement (new Contract No. CTR043241) between the Gila County Health and Emergency Management Department and the Arizona Department of Health Services to change the number of the contract from ADHS19-207418 to CTR043241 for the continued provision Gila County various nutritional services for the period of October 1, 2018, through September 30, 2023.
