| Approval of Amendment No. 7 to Professional Services Agreement No. 062813-Gila County Medical Examiner Mark Fischione |
| Background Information |
On January 7, 2014, the Board of Supervisors (BOS) approved Professional Services Agreement No. 062813 with Dr. Mark Fischione to provide medical examiner services to Gila County. The contract was issued for a period of one year in a not to exceed amount of $30,000 without prior written approval from Gila County for the contract term ending on January 6, 2015. Additionally, the contract allows for the option to renew the contract term for three additional one-year periods.
On June 24, 2014, the BOS approved Amendment No. 1 to the Agreement which increased the total contract by an additional $40,000 for a new total contract amount of $70,000 for the contract term ending on January 6, 2015.
On October 21, 2014, the BOS approved Amendment No. 2 to the Agreement which increased the total contract by an additional $30,000 for a new total contract amount of $100,000 for the contract term ending on January 6, 2015.
On December 16, 2014, the BOS approved Amendment No. 3 to the Agreement which renewed the contract for one additional year beginning January 7, 2015, through January 6, 2016; and the total not to exceed amount was revised from $30,000 to $70,000 for the contract term ending on January 6, 2016.
On September 1, 2015, the BOS approved Amendment No. 4 to the Agreement which increased the total contract by an additional $40,000 for a new contract amount of $110,000 for the contract term ending on January 6, 2016.
On January 19, 2016, the BOS approved Amendment No. 5 to the Agreement which increased the total contract by an additional $10,000 for a new contract amount of $120,000 for the contract term ending on January 6, 2016.
On February 2, 2016, the BOS approved Amendment No. 6 to the Agreement which extended the term of the contract from January 7, 2016, to July 6, 2016. This Amendment also served to increase the original dollar amount of the contract from $30,000 to $65,000. |
| Evaluation |
| As of Dr. Fischione's May billing, the sum of $65,000 has been expensed against the yearly contract which ends on July 6, 2016. Additional funds will be required to cover the remaining term of this contract. Amendment No. 7 will increase the current contract amount of $65,000 by an additional $30,000 for a new, not to exceed without prior written authorization total contract amount of $95,000 for the contract term from January 7, 2016, to July 6, 2016. |
| Conclusion |
| Additional funds are needed to cover the Medical Examiner's expense for the month of May and June 2016. As it is not possible to predict the costs that will be incurred during the course of the contract term, Amendment No. 7 has been issued to increase the contract amount from $65,000 to $95,000 for the remainder of the contract term, which expires on July 6, 2016. |
