| On June 9, 2015, the Board of Supervisors approved Professional Services Contract 041515-3 with Myers & Associates, PLLC whereby Myers & Associates, PLLC will provide legal services for indigent citizens as appointed by the Superior Court in Gila County for the period beginning July 1, 2015, to June 30, 2016. The original contract was executed for a not to exceed, without written authorization, amount of $51,000. |

| The number of cases that have been assigned to Myers & Associates, PLLC by the Superior Court in Gila County from July 1, 2015, to June 30, 2016, has caused the original contract amount of $51,000 to be exceeded. Amendment No. 1 has been issued to increase the contract amount by an additional $6,930 for an amended contract amount of $57,930 to cover the remainder of the contract term from July 1, 2015, to June 30, 2016. |
