Request/Subject
Amendment to the Housing Services Contract between Cenpatico Behavioral Health of Arizona, LLC, the Regional Behavioral Health Authority for the Arizona counties of Yuma, LaPaz, Gila and Pinal Counties and Housing Subsidy Administrator, Housing Services Contractor, to extend the contract ending date from 6/30/2015 to 9/30/2015. 
Background Information
The initial Housing Services Contract was approved by the Board of Supervisors on March 1, 2011, in an amount not to exceed $128,115.84 for the period of January 30, 2011, through June 30, 2015.
Evaluation
It is important for the Board of Supervisors to approve this Amendment between Cenpatico Behavioral Health of Arizona and Housing Subsidy Administrator, which will allow services to continue to be provided for administration and management of housing assistance and the provision of housing assistance services for qualified adults with serious mental illnesses. 
Conclusion
By the Board of Supervisors approving this Amendment, the terms of the Contract shall be extended to September 30, 2015. 
Recommendation
The Gila County Community Services Director recommends that the Board of Supervisors approve this Amendment to the Housing Services Contract.
Suggested Motion
Approval of an Amendment to a Housing Services Contract between Gila County and Cenpatico Behavioral Health of Arizona, LLC to extend the term of the Contract from June 30, 2015, to September 30, 2015.
