| Background Information |
| At the June 9, 2015 Regular Board of Supervisors meeting, the Board of Supervisors approved Professional Services Contract No. 041515-1 with Canizales Law, PLLC, whereby Carrie Canizales of Canizales Law, PLLC will provide legal services for indigent citizens as appointed by the Superior Court in Gila County for a period from July 1, 2015 through June 30, 2016. The original contract was executed for a not to exceed, without written authorization, amount of $41,000. |

| The number of cases that have been assigned to Ms. Canizales by the Superior Court in Gila County from July 1, 2015, through March 30, 2016, has caused the original contract amount of $41,000 to be exceeded. Amendment No. 1 has been issued to increase the contract amount by an additional $12,100 for an amended contract amount of $53,100 to cover the remainder of the contract term from July 1, 2015, to June 30, 2016. |
