| Approval of Amendment No. 1 to Contract No. 032712-1 with Visus Engineering Construction |
| Background Information |
On September 18, 2012, Gila County entered into a contract with Visus Engineering for adding some turn lanes at Russell Road, Besich Blvd. and Hope Lane, to improve roadway safety. The contract was issued through the Formal Bid process. Gila County requested proposals for this work and signed a contract with the lowest qualified bidder, Visus Engineering Construction, Inc.
The contract amount is $720,000. Included in the $720,000 contract amount is $75,000 for Force Account Work. (See attached Visus Bid Schedule, Item No. 109). The Force Account item is to be used as a contingency item for unforeseen extra work that may be required to complete the project.

| Evaluation |
There are ten (10) change orders totaling $121,157.86 that were extras related to the project, but not included in the bid items.
Change Order #1 was for New 4" PVC fiber optic conduits requested by Gila County- $17,666.97
Change Order #2 was for New waterline changes requested by Arizona Water Company - $19,549.05
Change Order #3 was for Lowering of a 1" gas sewer service which was in conflict of the new parking lot - $1,243.79
Change Order #4 was for Excavating and lowering of a 4" communication conduit, water service and relocate backflow preventor - $1,702.68
Change Order #5 was for Pothole and locate 4" communication conduit, and grade and backfill gas trench - $1,865.93
Change Order #6 was for Removal of existing concrete retaining wall located at the north end of Russell Road - $2,955.44
Change Order #7 was for Installing concrete cap over El Paso Natural Gas line - $22,137.00
Change Order #8 was to Remove an unforeseen subsurface condition, which included removal and backfilling of a cattle guard - $7,715.00
Change Order #9 was to Relocate water services from edge of roadway - $164.14
Change Order #10 was for the balance forward from Change Order #9 and final adjusted quantity overruns/underruns - $46,157.86
All but $46,157.86 of these change orders were included in the Force Account that was pre-approved in the contract. This bid was a unit price bid, not a lump sum bid. During construction the plans may not fit the actual construction so modifications have to be made. There are cost overruns and credits to the bid items totaling $46,157.86.
Attachment, entitled "Cost Overrun and Underrun Explanation" identifies the quantity overruns and underruns on some of the original bid items. |

| The original amount of Contract No. 032712-1 was for $720,000. Gila County Public Works would like authorization to increase the contract amount for Contract No. 032712-1 by $46,157.86, for a new total contract amount of $766,157.86, for cost overruns. If authorization is granted, an Amendment is attached to this agenda item for the Board's approval. |
