Amendment No. 1 Contract No. 080318-Supplemental Nutrition Assistance Program Education Support Services to Pinnacle Prevention.

Background Information

On October 30, 2018, the Board of Supervisors awarded a contract to Pinnacle Prevention for the period of November 1, 2018, through October 30, 2019.

Evaluation

Contract No. 080318 expires on October 30, 2019. The contract allows for three additional one-year periods at an annual cost of $165,000; therefore, staff recommends that the contract be renewed for one additional year, from September 1, 2019 through October 30, 2020.

Conclusion

To continue to meet grant requirements, Gila County is seeking to extend the award for Contract No. 080318 Supplemental Nutrition Assistance Program Education Support Services to contract support services to implement the Gila County Supplemental Nutrition Assistance Program Education (SNAP-Ed) food systems, active living, school health, and early childhood strategies in collaboration with Gila County Health and Emergency Management.

Recommendation

It is the recommendation of the Finance Director and the Health & Emergency Management Department Director that the Board of Supervisors extend contract No. 080318 with Pinnacle Prevention Corp to implement the Gila County Supplemental Nutrition Assistance Program Education (SNAP-Ed) food systems, active living, school health, and early childhood strategies in collaboration with Gila County Health and Emergency Services in the amount of $165,000; and amend the contract dates to align with the FFY of October 1, 2019 to September 30, 2020.

Information/Discussion/Action to approve Amendment No. 1 to Contract No. 080318 to approve the first extension to Contract No. 080318 for Supplemental Nutrition Assistance Program Education Support Services to Pinnacle Prevention Corp. in the amount of $165,000; amend the contract dates to align with the FFY of October 1, 2019, to September 30, 2020; and authorize the Chairman's signature on the Amendment.
