The Gila County Board of Supervisors approved Contract No. ADES15-089113 on June 23, 2015.

| Evaluation |
Amendment 2 to Contract No. ADES15-089113 will increase the reimbursement ceiling for Case Management from $301,731 to $315,971; an increase of $14,240. It will also increase the reimbursement ceiling for Community Services from $150,000 to $159,545; an increase of $9,545.
The cumulative reimbursement ceiling for the contract period July 1, 2015 through June 30, 2016 is $475,516.
An Itemized Service Budget for the services of Case Management (Attachment A) and Community Services (Attachment B) are revised and attached.
This Amendment will also add to the Contract in the section of Terms and Conditions, Levels of Service, section 32.7 which states: A Contractor may request and the Department may approve, the coordination and provision of emergency assistance outside the designated geographic service area for a specified time frame. No amendment to the contract shall be required.
Amendment No. 3 to Contract No. ADES15-089113 will revise the Case Management Service Budget for the period of July 1, 2015 through June 30, 2016. Per the Alert issued on October 9, 2015, the contract reimbursement ceiling is revised from $475,516 to $477,916.

| Conclusion |
| By the Board of Supervisors approving Amendment Nos. 2 and 3 to Contract No. ADES15-089113, the Gila County Community Action Program will have additional funding to provide case management services and community services to eligible residents of Gila County of which the intent is to assist the household in resolving crisis situations in order to move closer to self-sufficiency. |

| The Gila County Community Services Division Director recommends that the Board of Supervisors approve Amendment No. 2 and Amendment 3 to Contract No. ADES15-089113. |

| Information/Discussion/Action to approve Amendment Nos. 2 and 3 to an Intergovernmental Agreement (Contract No. ADES15-089113) between the Arizona Department of Economic Security and Gila County Division of Community Services, Community Action Program, revising the Itemized Service Budget for Case Management and Community Services whereby the cumulative reimbursement ceiling has been increased from $451,731 to $477,916, and to make some other minor contract changes for the period July 1, 2015, through June 30, 2016.
