Approval of Intergovernmental Agreement (Contract No. ADHS17-133182) Amendment No. 1 to increase our Public Health Emergency Preparedness grant award with the Arizona Department of Health Services in the amount of $55,870 for Zika Virus activities.
Background Information
The purpose of the Gila County Public Health Emergency Preparedness program is to readily prepare the staff and community for any public health emergency.  This includes staff training, development of disaster plans, organizing disaster exercises, and procuring emergency supplies and equipment.  The program exists to enhance all hazard strategic planning and direction, coordination and assessment, surveillance and detection capacities, risk communication and health information dissemination, telecommunications capabilities and education and training.

The Intergovernmental Agreement (Contract No. ADHS12-007886) was approved by the Board of Supervisors on December 6, 2011, in the amount of $155,256 for the contract period September 1, 2011, through August 30, 2012.

Amendment No. 1 to contract No. ADHS12-007886 changed the contract period from ending August 30, 2012, to June 30, 2012, in the amount of $155,256 and was approved by the Board of Supervisors at the January 24, 2012 meeting.

Amendment No. 2 to contract No. ADHS12-007886 amended the price sheet as a result of carryover of funds to reflect the amount of $239,056 and was approved by the Board of Supervisors at the May 22, 2012 meeting. 

Amendment No. 3 to contract No. ADHS12-007886 extended the contract period from July 1, 2012, to June 30, 2013, in the amount of $166,738 and was approved by the Board of Supervisors at the October 9, 2012 meeting. 

Amendment No. 4 to contract No. ADHS12-007886 extended the contract period from July 1, 2013, to June 30, 2014, in the amount of $175,104.66 and was approved by the Board of Supervisors at the October 22, 2013 meeting.

Amendment No. 5 to contract No. ADHS12-007886 extended the contract period from July 1, 2014, to June 30, 2015, in the amount of $200,419.10 and was approved by the Board of Supervisors at the September 2, 2014 meeting.

Amendment No. 6 to contract No. ADHS12-007886 amended the price sheet as a result of additional funding offered by the Arizona Department of Health Services to increase the award from $200,419.10 to reflect the amount of $215,419.10 and was approved by the Board of Supervisors at the May 26, 2015 meeting.

Amendment No.7 to contract No. ADHS12-007886 amended the price sheet as a result of additional funding offered by the Arizona Department of Health Services in the amount of $28,193 for a total contract amount of $243,612.10, and extends the contract period from April 1, 2015, to August 30, 2016 and was approved by the Board of Supervisors at the July 7, 2015 meeting. 

Amendment No. 8 to contract No. ADHS12-007886 amended the contract period from July 1, 2015, to June 30, 2016, in the amount of $200,419 and was approved by the Board of Supervisors  at the October 6, 2015 meeting.

Intergovernmental Agreement ADHS17-133182 begins a new contract period for the public health emergency preparedness grant extending our funding from July 1, 2016, to June 30, 2017, in the amount of $186,389.67 and was approved by the Board of Supervisors at the June 21, 2016 meeting.

Amendment No. 1 to contract ADHS17-133182 increases the grant amount by an additional $55,870 to provide Zika Virus community outreach, public education, and mosquito surveillance programs. 
Evaluation
This funding will allow the Gila County Health and Emergency Management Division, Department of Public Health Emergency Preparedness, to provide Zika Virus community outreach, public education, and mosquito surveillance programs; and, conduct case follow-up: follow up of women and infants in the pregnancy or birth defects registries. 
Conclusion
Approval of Amendment No. 1 to Contract No. ADHS17-133182 will allow funding to the Gila County Health and Emergency Management Division to continue to improve public health emergency preparedness services and support Gila County for Zika Virus community outreach, public education, and mosquito surveillance programs.
Recommendation
It is the recommendation of the Health & Emergency Management Division Director that the Board of Supervisors approve Amendment No.1 to Contract No. ADHS17-133182 with the Arizona Department of Health Services for the amount of $55,870 to support the continuance of the public health emergency preparedness grant services from July 1, 2016, through June 30, 2017.
Suggested Motion
Approval of Amendment No. 1 to an Intergovernmental Agreement (Contract No. ADHS17-133182) between the Arizona Department of Health Services and the Gila County Health and Emergency Management Division to accept an increase in the contract amount by $55,870 to provide continued Public Health Emergency Preparedness services and Zika Virus community outreach, public education, and mosquito surveillance programs.
