Amendment No. 6 to Contract No. DE126000-001 Arizona Department of Economic Security.

Background Information

1. Original Contract DE126000-001 was signed by the Board of Supervisors on April 19, 2011.
2. Unilateral Amendment No. 1 was added by Arizona Department of Economic Security (DES) on June 5, 2013.
3. Amendment No. 2 and 3 were signed by the Board of Supervisors on June 25, 2013.
4. Amendment No. 4 was signed by the Board of Supervisors on December 5, 2013.
5. Amendment No. 5 was signed by the Board of Supervisors on May 6, 2014.

Evaluation

Pursuant to the Department of Economic Security Special Terms and Conditions, Section 12 "Contract Term and Option to Extend," Contract No. DE126000-001 is extended for the period of January 01, 2015 through June 30, 2015.
The rates are not adjusted for the extension period and shall remain the same as for the previous contract period.

Conclusion

By the Board of Supervisors approving this Amendment, Contract No. DE126000-001 will be extended for the period of January 1, 2015, through June 30, 2015.

Recommendation

The Gila County Community Services Division Director recommends that the Board of Supervisors approve this amendment.

Suggested Motion

Approval of Amendment No. 6 to Contract No. DE126000-001 between the Arizona Department of Economic Security and the Gila County Community Services Division, Gila Employment and Special Training, whereby Amendment No. 6 extends the contract period from January 1, 2015, through June 30, 2015.
