Evaluation

The purpose of Amendment No. 12 is to:
Increase the reimbursement ceiling for the service Case Management from $319,381 to $344,053, which is an increase of $24,672; and to increase the reimbursement ceiling for the service Community Services from $150,000 to $162,338. This is an increase of $12,338, which includes $12,338 "Carry Forward" funding. The carry forward funding is not added to the cumulative reimbursement ceiling.

Conclusion

By approving Amendment No 12, the reimbursement ceiling for Case Management by Community Services will increase by $24,672 and the reimbursement ceiling for Community Services will increase by $12,338 to Contract No. DE111073001 with Arizona Department of Economic Security.

Recommendation

The Gila County Community Services Division Director recommends that the Board of Supervisors approve this amendment.

Suggested Motion

Approval of Amendment No. 12 to an Intergovernmental Agreement (Contract No. DE111073001) between the Arizona Department of Economic Security and the Gila County Division of Community Services, Community Action Services, to provide funding in the amount of $344,053 for the service of Case Management and $162,338 for the service of Community Services, to be used to provide assistance to eligible citizens residing in Gila County for the period beginning July 1, 2014, through June 30, 2015.
