Request/Subject

Funding Agreement No. 544-19 between the Arizona Department of Housing (ADOH) and the Gila County Community Services Department for Gila County Coalition to End Homelessness Planning.

Background Information

Funding Agreement No. 502-19 was approved by the Gila County Board of Supervisors on July 23, 2019.

A Certification Regarding Debarment and Suspension form has been added to the Agreement. Funding Agreement No. 502-19 has been renamed Funding Agreement No. 544-19.

Evaluation

Continuum of Care (COC) Planning Funds will be provided to the Gila County Community Services Department to act as the lead in forming a Gila County COC and to be responsible for activities, discussions, and meetings, directed at ending homelessness.

Conclusion

If Funding Agreement 544-19 is approved, the Gila County Community Services Department will act as lead in forming a Gila County COC and will be responsible for activities, discussions, and meetings directed at ending homelessness.

Recommendation

The Gila County Community Services Department Director recommends that the Board approve Funding Agreement No. 544-19.

Suggested Motion

Approval of a Funding Agreement between the Arizona Department of Housing and the Gila County Community Services Department to receive $5,000 of Continuum of Care Planning Funds to be used through December 31, 2019, to reflect a change in the contract number from 502-19 to 544-19 and to add a Certification Regarding Debarment and Suspension form.
