Intergovernmental Agreement (Contract No. ADHS14-053062) with the Arizona Department of Health Services.

Background Information

This contract replaces Contract No. ADHS11-004485. The purpose of this Intergovernmental Agreement (IGA) is for the Arizona Department of Helath Services (ADHS) to provide continued funding for the Gila County Nutrition Services Program of which the following program services are provided to qualified families: Women, Infant and Child (WIC), Breastfeeding Peer Counseling (BFPC), and Farmers Market Nutrition Program (FMNP).

Evaluation

Attachment A to the IGA is the Price Sheet of which prices have been established for the period October 1, 2013, through September 30, 2014, as follows: WIC services in the amount of $268,575, and BFPC services in the amount of $40,000. FMNP services are also included at $1.25 per WIC participant on as as-needed basis. This funding will allow Gila County to continue to provide nutrition education services to qualified families.

Conclusion

Without this funding, Gila County would be unable to provide nutrition education services to qualified families.

Recommendation

It is the recommendation of the Director of Health and Emergency Services that the Board of Supervisors approve this IGA with ADHS in the total amount of $308,575 to provide services for the period of October 1, 2013, through September 30, 2018. The Price Sheet is subject to change after September 30, 2014.

Suggested Motion

Approval of an Intergovernmental Agreement (Contract No. ADHS14-053062) between the Gila County Division of Health and Emergency Services and the Arizona Department of Health Services (ADHS) in the amount of $308,575 to allow for the continued provision of Nutrition Program Services for the period of October 1, 2013, through September 30, 2018. (This Contract replaces ADHS Contract No. ADHS11-004485.)
