Request/Subject

Amendment No. 7 to Contract No. HP652141-001 with Arizona Department of Health Services.

Background Information

This contract is for the care of HIV-positive individuals in a three-county area--Gila, Navajo, and Apache--to access medical services, utility assistance and emergency financial assistance.

Evaluation

Providing these services allows the HIV positive individual to access medical care and keep a place to live so that they continue to remain healthy and prevent loss of life for these citizens.

Conclusion

This contract is necessary for the ongoing care of HIV-positive persons.

Recommendation

The HIV Program Coordinator is requesting approval of the price sheet to increase the County's funding for this program for the remainder of the grant year.

Suggested Motion

Approval of Amendment No. 7 to Contract No. HP652141-001 between Gila County Division of Health and Community Services, Health Department, and the Arizona Department of Health Services to increase the price sheet from $58,523 to $62,146 for services provided to HIV-positive persons for the period August 1, 2010, to March 31, 2011.
